ANSWER

This shows patchy opacification more predominant in the upper lobes and in the periphery of the lung fields. The rest of the lung parenchyma looks normal. There is no adenopathy.

This radiological appearance is very characteristic of CEP. CT in hypersensitivity pneumonitis generally shows more diffuse pulmonary involvement and does not show an apical or peripheral predominance.

The patient is now 4 days in hospital and the parents note that he is much better. He has more energy and is coughing less.

WHAT IS THE SIGNIFICANCE OF THIS POINT?
