AP view: There is opacification of the lower part of the left hemithorax with widened pleural space, suggesting large pleural effusion. There is shift of the mediastinum to the right side. The right lung is clear. There is left subclavian central venous line.
Left lateral decubitus: Large left pleural effusion with fluid layering along the dependent portion of the left hemithorax.
At this point patient had a spike of fever to 38.2C° without any change in his respiratory status.
Blood work showed a white cell count of 1.7 x109/L with absolute neutrophil count of 1.2 x109/L, and severe lymphopenia with absolute lymphocyte count less than 0.5 x109/L, Hemoglobin: 10.4, Platelets: 200.Electrolytes, renal and hepatic profiles were all within normal limits. Blood and urine samples were submitted for bacterial stain and culture.
