ANSWER:


There are persistent ill-defined opacities and patchy infiltrations seen at the perihilar area and superior segment of the right lower lobe since the first chest x-ray done 2 years ago. The density of affected area seems to increase with time. There is no hilar adenopathy, pleural effusion, or pneumothorax. The cardiomediastinal contours are within normal limits. The bones and soft tissues are unremarkable.


What is your differential diagnosis at this stage?
