He had a right lung lavage again as that was the area of his CXR that was most dense. This time an LMA approach was used and lavaging a segment at a time. He also had another endoscopy which again showed no significant gastrointestinal pathology.

Despite this second lavage, with considerable fluid exchange, his clinical condition did not improve. By this stage there was doubt about the diagnosis.
