ANSWER:


We decided to admit the child to the hospital and asked for an ultrasound guided chest drain to be inserted. There was 400 ml of milky fluid withdrawn. Fluid analysis showed: cell count: 2300cells/µL with 16% lymphocytes, 63% neutrophils, and some monocytes; glucose 4.6mmol/L; albumin 10g/L; LDH 124U/L; triglyceride 25.3mmol/L; cholesterol 1.53mmol/L. Centrifugation resulted in grossly lipemic appearance. Bacterial, viral, fungal as well as acid fast stains were negative and cultures were submitted.

Do you agree with the diagnosis of chylothorax given the above mentioned pleural fluid analysis? What is with and against this diagnosis?
