On physical examination he looked unwell but was not in obvious distress. His heart rate was 100, respiratory rate 28-32, O2 saturations 98% on 40% FiO2 via nasal prongs (he dipped quickly to 82% off supplementary O2) and blood pressure 100/60.

There were scattered mouth ulcers and his eyes,ear,nose and throat exams were normal. There was no cervical lymphadenopathy.

His chest exam revealed fair air entry with bilateral diffuse coarse inspiratory crackles but no wheeze. His abdomen was soft and non-tender with no significant organomegaly.

His cardiac exam showed a well perfused child, normal heart sounds with no added sounds nor murmurs.

The musculoskeletal exam showed dystrophic finger and toe nails as well as skin changes consistent with Dyskeratosis Congenita.

Laboratory results showed a haemoglobin of 89, white cell count of 4.9 and platelets of 69,000. Electrolytes were normal. INR/PTT were normal. A capillarized blood gas showed a pH of 7.49 and a pCO2 of 33..

A chest x-ray was ordered
