PRESENTING FELLOW

REASON FOR REFERRAL
A 14 month old previously well male who presented to the emergency department with 6 days of cough, and two days of fever and irritability. He had some post-tussive vomiting, and occasional regurgitation following feeds. He was not drinking well in the last two days.CASE PRESENTATION
He was born at term, and weighed 3685 grams. He
had thrush at two months of age, and bronchiolitis with otitis media at
six months. The family history is remarkable for asthma in a paternal
aunt and grandmother. Nutritional and developmental histories are normal
and his vaccinations are up to date. He is on no medications and has no
known allergies.
PHYSICAL EXAM: T° 38.2, RR 52, HR 160, BP normal. O2
sat 96% on room air.
He was dehydrated, and lethargic. He was indrawing, and had decreased
air entry to the left lower lobe area with inspiratory crackles.
