ANSWER:


We ordered a bubble ECHO cardiagram which came back suggestive of artriovenous malformations (AVM). Subsequently, pulmonary angiography was done and it demonstrated numerous bilateral distal AVM's too small for ablation.


We were left with a rare pulmonary complication in a rare disease. Based on a published case study, he was given nifedipine 10mg po twice daily in order to treat his AVM's. (Pediatr Cardiol 21:493-496, 2000.)

His O2 requirement came down to 1.5-2L/ min. Seven months later, he reported increased exercise tolerance and an improved overall sense of well-being.

Recently, LF was admitted with an upper gastrointestinal bleeding that was related to AVM's in the GI tract.
