Answer

The left lower lobe is superimposed on one large and few small cystic, air containing cavities with no air-fluid levels. The left hemidiaphragm is intact. There is a mild midline shift to the right. However, the right lung is normal. An incidental note is made of a cystic lesion in the tip of the spleen.
