ANSWER
There is a mildly elevated titer for micropolysporia faenia (the fungus responsible for farmers lung) However 50% of individuals exposed to a particular antigen will develop elevated precipitating antibodies and hypersensitivity pneumonitis will only develop in a small percentage of these. In an evaluation of cases reported in the literature in children 89% of those with hypersensitivity pneumonitis will have positive precipitins. The result although mildly elevated in this case does not give support to a diagnosis of hypersensitivity pneumonitis. The following CT scan of chest was performed.
